Personalized Medicine ServIce for Primary Care Community Center
The main objective of the PErsonalized Medicine Service for Primary Care Community Center (PEMS-PC3) project was to design, deploy and verify the clinical efficacy and economic impact of care pathways created for the selected diseases, namely COPD, type 2 diabetes and high blood pressure. These care pathways will consist of a set of programs based on a personalized medicine model, in which primary care centers and community centers coordinate, to give support and follow-up to patients who use them.
This project designed, implemented and deployed a personalized service that was tested in a community center in China. This pilot service included the following elements / activities: service platform; shared medical history; patient registration; personalized health care plans; clinical and economic validation.
The implementation of PEMS-PC3 aimed to:
– Provide information to managers and policy makers about the health profile of its inhabitants to improve the planning of future health services.
– Encourage citizens to achieve a healthier lifestyle thanks to therapies for reducing tobacco dependence, improving daily physical activity and nutritional habits.
– Promote early detection and a more accurate diagnosis of COPD, type 2 diabetes and high blood pressure, facilitating earlier intervention.
– Improve the management of chronic diseases of patients with COPD, type 2 diabetes and high blood pressure to slow progression and improve outcomes.
